Greg Marchand
Greg Marchand, M.D. é um médico americano especializado em ginecologia e obstetrícia inventor de dois procedimentos cirúrgicos. O primeiro é relacionado à esterilização das mulheres a partir da retirada das tubas uterinas, reduzindo desse modo o risco de câncer no ovário,[1][2] e o segundo é a realização de cerclagem cervical via laparoscopia com o objetivo de prevenir o nascimento prematuro.[3] Marchand é Professor associado ao Colegiado de Medicina da Faculdade de Medicina Osteopática na Universidade do Arizona, em Glendale, Arizona e também na Faculdade de Medicina Osteopática de Kirksville em Mesa, Arizona. Além disso, Marchand tem sido foco de críticas no campo cirúrgico devido à sua posição favorável às técnicas de Morcelamento.[4][5]

Biografia editar
Marchand é originalmente de Johnston, Rhode Island, e cursou a faculdade de medicina na Spartan Health Sciences University, no Caribe.[17] Concluiu sua formação em obstetrícia e Ginecologia na Universidade do centro de Ciências da saúde do Tennessee em Memphis, Tennessee.[17]
Técnicas cirúrgicas inventadas editar
A técnica cirúrgica "Salpingectomia Marchand” foi desenvolvida por Marchand logo após a Escola Americana de Obstetrícia e Ginecologia (American College of Obstetricsand Gynecology) divulgar o parecer do Comitê #620.[18] A opinião do comitê mudou consideravelmente a cirurgia ginecológica por indicar a Salpingectomia (remoção das trompas de Falópio) em casos onde a ligadura de tubas uterinas (trompas de Falópio) era recomendada.[18] Marchand originalmente descreveu e publicou a técnica no dia 24 de outubro de 2017, no Congresso Clínico da Escola Americana de Cirurgiões (Clinical Congress of the American College of Surgeons) em San Diego, California.[1][2]
Dr. Richard Demir e Marchand são os autores de uma técnica sem nome para a colocação do laparoscópio a partir da cerclagem cervical, afim de prevenir a insuficiência cervical em casos de nascimentos prematuros. Um vídeo descrevendo esta técnica foi posteriormente publicado no 32 º Congresso Internacional Britânico de Obstetras e Ginecologistas, em Belfast, Reino Unido em 20 de junho de 2010.[3][19] Esta técnica foi adicionada à lista publicada de competências essenciais na escola de medicina de Nova Jersey da Universidade de Rutgers em 2018.[20]
Recordes cirúrgicos editar
Marchand, juntamente com o Dr. Richard Demir, removeram o maior útero já removido através de uma histerectomia completamente laparoscópica, em 2007.[21] O útero tinha 3200 gramas, e o recorde mundial foi verificado e reconhecido pelo Guinness World Records em 2008.[10] Um sumário que descreve a técnica cirúrgica e os detalhes foi publicado mais tarde na edição do 4º trimestre do jornal 2010 da sociedade de cirurgiões de Laparoendoscopia.[22] Este recorde foi mais tarde quebrado em 2009 pelo Dr. Rakesh Sinha do hospital BEAMS em Mumbai, Índia, com um útero de 4100 gramas.[23]
Em 2015, Marchand removeu uma massa ovariana de 17 centímetros de um paciente, que inicialmente se pensou ser benigno.[6][7][8][11] A patologia intraoperativa revelou que a massa era maligna, e o Dr. Snehal Bhoola, oncologista ginecológico, ajudou Marchand a terminar a Cirurgia de Estadiamento de Câncer Ovariano completamente a partir de técnicas laparoscópicas. Essa foi a massa ovariana de maior volume já retirado utilizando exclusivamente o procedimento de Estadiamento Cirúrgico por meios completamente laparoscópicos. Este registro foi verificado e reconhecido pela World Record Academy em 2017.[11] O recorde recebeu críticas significativas porque embora a massa não fosse conhecida por ser cancerosa antes da remoção, foi removida usando laparoscopa de morcelamento (morcellation), a qual tem demonstrado aumentar o risco de propagação de um câncer em todo o abdômen.[24][25]
Em 2018, Marchand realizou uma histerectomia laparoscópica de porta única através da menor incisão já relatada, 11mm.[13][14] Este registro foi verificado e reconhecido pela World Record Academy em 2018. A Academia relatou que o registro anterior tinha sido mantido pela Cleveland Clinic e era 15mm.[13]
Saúde pessoal editar
Em 2010, Marchand foi diagnosticado com um coriocarcinoma, um cancro testicular agressivo[6] Marchand submeteu-se à cirurgia e aos tratamentos no mesmo ano e relata que esteve na remissão desde então.[7] Marchand cita sua própria batalha pessoal como sua inspiração para a criação de técnicas cirúrgicas para tratar o câncer em entrevistas e publicações.[9]
Morcelamento editar
Dois dos registros de Marchand envolvem o morcelamento (morcellation), um processo cirúrgico que tem sido mostrado em estudos com um potencial de espalhar células cancerosas por todo o paciente.[24] Marchand recebeu críticas significativas após seu segundo recorde mundial em 2017, devido à este recorde, ao contrário do primeiro, ter sido realizado após o aviso da FDA sobre o perigo da técnica de morcelamento, em 2014.[26]
